Reconstrução microcirúrgica de mandíbula com retalho osteocutâneo de fíbula, com auxílio de modelo 3D, em paciente vítima de trauma: relato de caso / Mandible microsurgical reconstruction with osteocutaneous fibular flap, with the assistance of a 3D model, in a trauma patient: case report

Trata-se de um relato de caso de reconstrução microcirúrgica de mandíbula com retalho osteocutâneo de fíbula em um paciente de 16 anos, vítima de agressão com projétil de arma de fogo e trauma contuso em face, acarretando em fratura cominutiva de mandíbula, com perda de elementos dentários e lesões extensas de partes moles. Após tratamento da fase aguda, optou-se pela reconstrução microcirúrgica com retalho livre de fíbula, devido à extensão do defeito mandibular e possibilidade de reabilitação dentária. O uso de modelos tridimensionais da mandíbula e fíbula foram excepcionalmente úteis para tornar possível o melhor planejamento cirúrgico das áreas de osteotomia, com diminuição do tempo de isquemia do retalho da cirurgia.

This is a case report of the mandible microsurgical reconstruction with osteocutaneous fibular flap in a 16-year-old patient. A victim of an assault with a firearm projectile and blunt trauma to the face, resulting in a comminuted mandible fracture, loss of dental elements, and extensive soft tissue injuries. After treatment of the acute phase, we chose microsurgical reconstruction with a free fibula flap due to the mandibular defect's extension and the possibility of dental rehabilitation. The use of three-dimensional models of the mandible and fibula was exceptionally helpful in making the best surgical planning of the osteotomy areas, decreasing the surgery flap's ischemia time.

Incidência e etiologia das fraturas de face na região de Venda Nova - Belo Horizonte, MG - Brasil / Incidence and etiology of face fractures in the Venda Nova region - Belo Horizonte, MG - Brazil

O propósito deste trabalho é analisar a frequência, incidências e etiologia das fraturas de face, na região de abrangência do Hospital Risoleta Tolentino Neves, em Belo Horizonte ? MG entre janeiro de 2007 e dezembro de 2009. O osso nasal foi o mais comumente fraturado, seguido da mandíbula, malar, arco zigomático e maxila.

The purpose of this paper is to analyze the frequency, incidence and etiology of face fracture, in the region of the Hospital Risoleta Tolentino Neves, in Belo Horizonte - MG, between January 2007 and December 2009. The nasal bone was the most commonly fractured, followed by the jaw, cheekbone, zygomatic arch and maxilla.

Fratura cominutiva grave de mandíbula por arma de fogo: relato de caso / Severe gunshot comminuted mandible fracture: case report

Relata-se o caso de paciente atendido no Pronto-Socorro com fratura exposta cominutiva grave de mandíbula, com perda óssea importante causada por projétil de arma de fogo. Foi submetido àa operação de urgência, sendo necessário o uso de placas de fixação e reconstrução. Apresentou evolução demorada, com período de internação hospitalar de 28 dias. À alta, apresentava deformidade facial e dificuldade na movimentação da mandíbula e articulação de palavras. Devido ao tratamento com cirurgia precoce e ao uso de recursos adequados, o paciente obteve bons resultados estéticos e funcionais no controle ambulatorial.

Mandible fractures are among the most common lesions encountered in the trauma centers. They are usually caused by assault or gunshot and the risk factors include low socioeconomic level, drug abuse and unemployment, being male young adults the most frequent victims. This report is about a patient who was brought to the emergency room with a severe gunshot comminuted mandible fracture. He was submitted to an early surgical repair, in which fixation and reconstruction plates were needed. There was a 28 day in-hospital stay and the patient showed facial deformation and difficulty in word pronunciation at discharge. Due to the aggressive management with early surgical repair and adequate resources the patient presented with good functional and aesthetical results in the follow up.
